Personal Data Protection Act Consent Form
因應個人資料保護法(下稱個資法)及本會個人資料管理規定，本會在向您蒐集個人資料之前，依法向您告知下列事項：
In compliance with the Personal Data Protection Act (hereinafter referred to as the PDPA) and the personal data management regulations of our association, we are required by law to inform you of the following before collecting your personal data:
一、蒐集目的及類別 (Purpose of Collecting and Categorizing Data)
為提供本活動各項通知服務、寄送本會相關活動訊息及內部管理使用之蒐集目的，須獲取您下列個人資料：姓名、電話、E-mail或其他得以直接或間接識別您個人之資料等，以本協會所實際蒐集為準。
To provide various notification services for this event, we need to collect the following personal data from you, including name, phone number, email and any other information relating to you directly and indirectly. Based on the data that are collected for the events, they are for internal use only.
二、個人資料利用之期間、地區、對象及方式 (The duration, geographical location, authorized individuals, and processing methods of personal data utilization)
您的個人資料僅供本協會及活動協辦單位於上述蒐集目的之必要合理範圍內加以利用，另做其他用途。
Your personal data will be used solely by our association and event co-organizers within the necessary and reasonable extent of the stated purposes. It will not be used for any other purposes.
三、當事人權利行使 (Exercise Your Right as Data Subject)
依據個資法規定，您可向本會請求更正、停止蒐集/處理/利用或刪除您的個人資料。有關各項通知服務之停止，您可來信或於上班時間聯繫本協會。
According to the regulations of the Personal Data Protection Act, you have the right to request a correction of personal information; terminate your information collection process; deny our association to handle and utilize your personal information or ask for your personal data to be deleted from our association. For the termination of various notification services, please send us an email or contact us during office hours.
